Yes - I did make the frame myself. This page outlines making the frame:

As for the stick change (gear lever), it is possible to weld a steel bar 
with a knob on the end of it, directly to the gear change shaft coming 
from the engine. This is the easiest method. When you get the engine, you 
will see the foot change lever. Simply weld a longer piece of steel bar to 
this foot change lever, and you can use your hand to operate the gears.

A more complicated way is to setup a gear change linkage. This page
